East Lyme Emergency Management is working with our Senior Center Director and Ledge Light Health District to identify residents unable to leave their home for medical reasons. It’s extremely important that we identify the homebound members of our community to ensure they can receive the COVID-19 Vaccine when supplies become available and scheduling for in-home vaccinations can begin.
Starting this week, the Senior Center Staff will be making phone calls to East Lyme homebound residents that they are currently aware of. Additionally, they will reach out to residents whom have registered with the Town’s Emergency Management office, and indicated their need for assistance in the event of an emergency evacuation. If you know of a homebound resident that is in need of this in-home vaccination service, please share this information with them.
To qualify, for in home vaccinations, individuals must:
- Be homebound (unable to physically leave their home)
- Live in geographical bounds of the Town of East Lyme
- Meet current eligibility requirements outlined in the state’s COVID-19 vaccine rollout plan.
- Vaccinations will also be provided for one primary caregiver for the eligible homebound resident, providing they meet the eligibility requirements at the link provided above. The caregiver must also be in the home at the residents scheduled appointment time.
Forms to required for the COVID-19 Vaccine Home Visiting Program are available below. Those without Internet access can call the East Lyme Senior Center at 860-739-5859 to provide their information.
The homebound vaccination program is not available to individuals who have transportation challenges. Any HUSKY A, C, D and limited benefit members who are eligible for non-emergency medical transportation can call Veyo at 855-478-7350 — at least 48 business hours in advance — to arrange for transportation to and from vaccine appointments at any time. The service is free for such members.
